Required Forms

Each of the forms on this page is essential in helping us to prepare for your arrival at Hollins. Please note the due dates for submitting them to us over the summer.

Advising Questionnaire: This form will help your academic advisor learn about your academic and extracurricular interests. Please complete this online form by June 17, 2005.

New Student Housing Information Form: This form will help the Housing and Residence Life staff assign you a room and a roommate. Please complete this online form by June 17, 2005.

"Big Sister" Program Questionnnaire: Hollins has a unique big sister program. Please fill out the Big Sister Application form so that we can match you up with the best big sister for the upcoming year. The identity of your “Goombah” (companion, person acting as an advisor or mentor) will be a secret until about the third week of the fall term. The information in your form will be shared with your big sister so she can learn about what you like to do outside of class. Please complete this online form by June 17, 2005.

Myers Briggs Type Inventory: All first-year and transfer students must take the online Myers Briggs Personality Inventory which will be used by the Student Affairs staff to help with roommate selection and teambuilding exercises during orientation. The results of your inventory will be shared with you and then kept on file in the Career Development Center. This exercise can be very useful later on with internship and career planning. Please complete this online form by June 17, 2005. 

Health and Immunization Records: Hollins requires all residential students to provide health and immunization records and proof of insurance. Please print the forms under this link, fill them out, and mail them to us by the August 1, 2005 deadline. Students will not be allowed to register for fall term classes if they have not submitted their health forms.

Religious Preference Form: This optional form is requested by the Hollins University chaplain to find out about your spiritual interests and needs. Please complete this online form by June 30, 2005.

Release of Information Form: This optional form grants Hollins University permission to publicize your accomplishments in your local newspaper. Please complete this online form by June 30, 2005.
