This site is developed as a repository of survey instruments designed to assess library services. A project of the LAMA LOMS Planning and Evaluation of Library Services [PELS] Committee, the L-SAC has been created to fill a need for sharing and refining the instruments used to assess library services. You are invited to browse the instruments and accompanying documentation and adapt the instruments for local use. You are encouraged to submit surveys used at your institution as well as surveys you have adapted for local use and would like to share with others.
